• Eat foods that are low in saturated fats, trans fats, cholesterol, salt and added sugars. Choose lean meats, poultry, fish, nuts, fat-free or low-fat milk and milk products.

History:

* Pulmonary TB: Typical symptoms of pulmonary TB include a productive cough, fever, and weight loss. Occasionally, patients may presents with hemoptysis or chest pain. Other systemic symptoms include anorexia, fatigue, or night sweats.

* Tuberculous meningitis: Patients may present with a headache that is either intermittent or persistent for 2-3 weeks. Subtle mental status changes may progress to coma over a period of days to weeks. Fever may be low-grade or absent.

* Skeletal TB: The most common site of a involvement is thae spine (Pott disease). Symptoms include back pain or stiffness. Lower extremity paralysis occurs in as many as half the patients with undiagnosed Pott disease. Tuberculous arthritis usually involves only 1 joint. Although any point may be involved, the hip of the knee is affected most commonly, followed by the ankle, elbow, wrist, hip or the knee is affected most commonly, followed by the ankle, elbow wrist and shoulder. Pain may precede radiographic changes by weeks to months.

* Genitourinary TB: Reported symptoms include flank, pain, dysuria, or frequency. In men, genital TB may present as epididymitis or a scrotal mass. In women, genital Tb may mimic pelvic inflammatory disease. TB causes approximately 10% of sterility in women worldwide and approximately 1% in industrialized countries.

* Gastrointestinal TB: Any site along the gastrointestinal tract may become infected. Symptoms are ferable to the site infected, to include the following: nonhealing ulcers of the mouth or anus; difficulty swallowing with esophageal disease; abdominal pain mimicking peptic ulcer disease with stomach or duodenal infection; malabsorption with infection of the small intestine; and pain, diarrhea, hematochezia with infection of the colon.

* Tuberculosis lymphadenitis (scrofula): The most common site is in the neck along the sternocleidomastoid muscle. It usually is unilateral. It usually is unilateral, with little or no pain. Advanced disease may suppurate and form a draining sinus.

* Cutaneous TB: Direct inoculation may result in an ulcer or wartlike lesion. Contiguous spread from an infected lymph node typically results in a draining sinus. Hematogeneous spread may result in a reddish brown plaque on the face or extremities (lupus vulgaris) or tender nodules or abscesses.

Physical: Finding upon physical examination depend on the organs involved.

* Patient with pulmonary TB have abnormal breath sounds, especially over the upper lobes or areas invovled.

* Sign of extrapulmonary TB differ depending on the tissues involved. Signs may include confusion, coma, neurologic deficit, chorioretinitis, lymphadenopathy and cutaneous lesions (as described above).

* Postnatal TB is contraced via the airbone route. The most common findings are adenopathy and a lung infiltrate. However, the chest radiography findings can be normal in infants with disseminated disease. Many experts increase treatment time to 9 or 12 months because of the possible impaired immune system in children younger than 12 months. 2 1/2 c. graham cracker crumbs

1/4 c. nuts

Combine milk and cocoa in saucepan. Beat well. Add oleo, sweetner, salt. Bring to boil. Remove from heat. Stir in remaining ingredients except 1/4 cup graham crackers. Cool about 15 minutes. Divide mixture into 32 balls. Roll in remaining cracker crumbs and chill.

An estimated 97 million adults in the United States are overweight or obese. The distinction between overweight and obese is a matter of quantity. Obese individuals simply have more body fat. Both these physical conditions significantly increase morbidity from many well known chronic health conditions. Following are a variety of common conditions found in overweight and obese indiviuals:

1) Metabolic Syndrome X. A cluster of cardiac symptoms which increase the risk for a heart attack or stroke.

5) Loss of muscle tone and bone density due to inability or refusal to exercise. As time goes by, there is muscle loss and the mineral content of the bones decreases. Loss of muscle tone leads to loss of strength to do simple daily activities. Loss of bone density could lead to dangerous falls and fractures.

6) High Blood Pressure (HBP). Normal Blood Pressure is 120/80 (mm hg) and represents the pressure within your arterial system. Overweight and obese individuals often have readings above 140/90 which is high. HBP is known as the "silent killer" because it has no symptoms.

7) High Cholesterol Levels. Cholesterol profiles tend to be suboptimal. This can lead to other forms of heart and circulatory disease.

8) Coronary Artery Disease. This is caused when plaque builds up inside the arteries that supply the heart. Restricting the blood supply could cause a heart attack.

9) Arteriosclerosis. This involves a hardening of the arteries, making them hard and brittle often leading to a stroke.

10) High blood insulin levels. This is a risk factor for diabetes and heart disease.

11) Gallstones and other gall bladder related problems.

12) Psychological imbalances such as depression, anxiety, eating disorders and low self esteem.

13)Sleep apnea. This can lead to a heart attack by restricting oxygen to the brain and cause other respiratory difficulties.

14)Urinary control problems such as incontinence.

Coronary heart disease (CHD) is the most common cause of mortality and morbidity in the elderly. In western countries, it accounts for 80 – 85 percent of all cardiac deaths in older people.

When a person ages, his or her cardiovascular system will undergo some normal and expected changes. These can be age-associated changes in cardiac anatomy; age-associated changes in cardiovascular physiology; and age-associated changes in pharmacokinetics and pharmacodynamics.

The presentation of CHD may be altered by these changes together with other age-related changes in the kidneys, brain, and musculoskeletal systems. Ultimately, the diagnosis and hence the treatment of heart disease in elderly may become more complicated.

Various conventional risk factors for CHD among younger people such as smoking, hypertension, hyperlipidemia and diabetes mellitus remain important factors for elderly. Furthermore, age is a powerful and independent risk factor for CHD. The risk increases when men reach 55 years old and above and women reach 65 years old and above.

Females have a much lower risk of CHD compared to males, but this gender differential diminishes as age advances. For example, the event rate is 1:5 in favor of females at the age of 35, but by the age of 70, the ratio is almost 1:1.

Symptoms such as breathlessness, giddiness or palpitation are quite common for the CHD in the elderly. Sometimes, musculoskeletal problem in the elderly may prevent the patient from complaining of exercise related angina. Instead of complaining of chest pain during a heart attack, an elderly patient may develop difficulty in breathing or symptom of stroke. Silent heart attack is also common. As reported in the Cardiovascular Health Study and the Framingham Heart Study, some 40 percent of unrecognized heart attack was found in those aged between 75 and 84 years.

In the management of heart disease, some diagnostic procedures such as treadmill exercise may not be suitable because of the associated joint problems in the elderly. It is also more risky to perform coronary angiography and coronary angioplasty procedures for the elderly patients with the presence of diminished kidney function. Following a heart attack, the in-hospital mortality and risk of subsequent reinfarction or its complications are all increased in the elderly patients. The use of clot busting medication (thrombolytics) has an increased risk of cerebral hemorrhage in the very elderly. Some age-associated changes in kidney or gastrointestinal function may require reduction in the usual drug dosages when heart medications are used in older patients.

The combination of an eating disorder and pregnancy is a dangerous one for both the mother and the child. Eating disorders can have a great deal of effects on the mother's body. There are several things that a person with an eating disorder can do to have a healthy pregnancy including:

• be in counseling/treatment for the eating disorder to deal with the eating disorder before becoming pregnant

• be back to a normal body weight and eating a healthy diet including the use of prenatal vitamins

• stop compensatory behaviors, such as binging, purging, excessive exercise or laxative use before pregnancy occurs

• consult with a doctor for regular checkups and make sure to let your OB/GYN doctor know about the eating disorder at your first visit if he/she is not aware of it already

• consult with a nutritionist to create healthy eating habits and diets that are well balanced and nutritious for both you and the baby

• be in counseling to deal with physical and emotional changes that will occur during and after the pregnancy

• Continue healthy eating after pregnancy in order to have the ability to breastfeed, which can have benefits for both the mother and the child

Diabetes:

Diabetes is a chronic condition in which body does not produce or produce less insulin compared to healthy individual or body develops resistance to insulin (body does not respond to insulin).

Most foods that we consume finally enter into the blood in form of carbohydrate, fat & protein. From blood, these nutrients enter into cells. Insulin is required for this entry of glucose into cells. Body cells utilize carbohydrate & fat for energy & growth.

It started out with him feeling excessive thirsty. He had to drink Coca Cola because drinking plain water did not seem to quench his thirst. He could drink up to 8 cans a day to feel satisfied. He became unwell, lethargic, and very bad-tempered. Finally, he had to go back to the doctor who had to send him to hospital to consult a specialist because his blood sugar level was already dangerously high.

During the course of treatment from the specialist, he was told what he should eat and what he should not. Instead of eating every time he felt hungry, he was asked to have 6 meals a day – a proper breakfast, a fruit at 10 am, lunch, wholemeal bread for tea, dinner, and a glass of milk for supper. He was also advised to take up brisk walking regularly but he should not jog at his age as his heart may not be able to take it. So he brought meals from home to his work place.

He attributed the credit to his wife who makes sure that he eats the foods that are low in salt, sugar, and fat contents and stays away from sweetened drinks. She even wakes up early to ensure he is suitably dressed for his morning brisk walks at about 3 am.

There are many other diabetics like him that watch their diet, exercise regularly, and take medication prescribed by doctors if necessary and managed to bring their blood sugar down to the healthier levels.
